Indications
Renocal Plus is used to treat high phosphate levels in the blood, which are often seen in patients with kidney problems undergoing dialysis (hemodialysis or peritoneal dialysis). This medicine is for adults.
Pharmacology
Renocal Plus works by binding to phosphate in food and forming low-solubility calcium and magnesium phosphate salts in the stomach. These salts are then excreted in the stool. Calcium acetate is effective at a pH of 6-8, making Renocal Plus suitable for people with low stomach acid.
Dosage & Administration
Adults: Take 3 to 10 tablets per day, depending on the phosphate levels in the blood. The daily dose should be divided into meals (usually 3 times a day). The starting dose is 3 tablets a day, and if necessary, the dose can be increased to a maximum of 12 tablets a day.
Pediatric patients: Safety and effectiveness in children and teenagers have not been established. Renocal Plus is not recommended for patients under 18 years of age.
How to take: Take the tablet with meals for the best effect. Do not crush or chew the tablet. If the tablet is too large to swallow, break Renocal Plus along the score line before swallowing. Do not take other medicines within 2 hours before or 3 hours after taking this tablet.
Interaction
To avoid interactions, do not take any other oral medicines within 2 hours before or 3 hours after taking this tablet.
This medicine can affect the absorption of certain antibiotics (e.g., tetracyclines, doxycycline), some cephalosporins, quinolones, and other drugs like iron, zinc, ketoconazole, and others. Be cautious if using this medicine with other treatments.
Magnesium can reduce the absorption of digoxin and nitrofurantoin, and may affect the effectiveness of certain drugs like penicillamine.
Calcium absorption is increased by vitamin D and decreased by some diuretics, so monitor calcium levels if using both.
Contraindications
Renocal Plus should not be used by patients who have:
- Low phosphate levels in the blood
- High calcium levels (with or without symptoms)
- Elevated magnesium levels
- Heart block (AV-block III°)
- Myasthenia gravis
- Allergy to any of the ingredients in the medicine
Side Effects
Possible side effects include:
- Digestive issues like nausea, constipation, diarrhea, and bloating
- High calcium or magnesium levels in the blood
Pregnancy & Lactation
There is limited information on the use of this medicine during pregnancy. Renocal Plus should not be used unless necessary for the woman’s condition. Renocal Plus is passed into breast milk, so breastfeeding is not recommended during treatment.
Precautions & Warnings
This medicine should be used with caution, especially in patients with kidney issues or high phosphate levels. Monitoring of calcium, magnesium, and phosphate levels is necessary. Long-term use may cause high magnesium levels, so regular check-ups are important.
Patients on a calcium- or magnesium-containing antacid should consult their doctor before using this medicine. Renocal Plus is not recommended for children under 18 years of age.
Overdose Effects
In case of overdose, symptoms may include muscle weakness, digestive problems, confusion, and coma. High magnesium levels can cause serious issues such as low blood pressure, paralysis, and coma. Severe calcium overdose can lead to kidney problems, heart issues, and confusion.
If overdose occurs, seek medical attention immediately. Treatment may involve reducing the magnesium levels or stopping the medicine.
