Indications
Repanid is used along with diet and exercise to help lower blood sugar in patients with type 2 diabetes when diet and exercise alone are not enough. Repanid can also be combined with Metformin for patients whose blood sugar is not controlled by diet, exercise, or either of these medications alone.
Pharmacology
Repanid works by binding to specific receptors on the cell membrane, which causes the potassium channels to close. This leads to a change in the cell’s charge, allowing calcium to enter the cell, which stimulates the release of insulin.
Dosage & Administration
Adults: If you haven’t been treated before or have an HbA1c of less than 8%, the starting dose is 0.5 mg before each meal.
If you have been treated with other blood sugar-lowering drugs and have an HbA1c over 8%, the starting dose is 1 or 2 mg before each meal.
Repanid should be taken right before or up to 30 minutes before meals.
The dose can be adjusted every 1 to 2 weeks based on how well Repanid works, with a maximum of 16 mg per day, taken as a single dose of up to 4 mg.
Interaction
Taking Repanid with other medications may require dose adjustments. Some drugs can either lower or increase its effectiveness. For example:
- Drugs that slow down the breakdown of Repaglinide (like azole antifungals and macrolides) may increase its effect.
- Medications that speed up its breakdown (like Rifampin and Phenobarbital) may reduce its effect.
- Repanid doesn’t significantly affect drugs like Digoxin, Theophylline, or Warfarin.
- Certain drugs that are highly protein-bound (like NSAIDs) can increase its blood levels, which may increase the risk of low blood sugar.
- Other drugs, like salicylates and sulfonamides, may also increase the risk of low blood sugar when taken with Repaglinide.
Contraindications
Repanid should not be used by patients who have:
- Diabetic ketoacidosis, with or without coma.
- Type 1 diabetes.
- Known allergies to this drug or any of its ingredients.
Side Effects
The most common side effect is low blood sugar (hypoglycemia). Other side effects can include:
- Upper respiratory infections
- Diarrhea
- Constipation
- Nausea and vomiting
- Skin rashes and hives
Pregnancy & Lactation
Repanid has not been tested for safety in pregnant women. Repanid should only be used during pregnancy if necessary. Repanid is not known if Repanid passes into breast milk. Since some medications can cause serious side effects in infants, you should consult with your doctor to decide whether to stop breastfeeding or stop taking this medication, depending on what is best for you.
Precautions & Warnings
Insulin should be used during illnesses like heart attack, coma, or infection, and during surgery. All blood sugar-lowering medicines can cause low blood sugar. To reduce this risk, take Repanid with meals.
Overdose Effects
Taking up to 80 mg of this drug did not cause significant issues beyond lowering blood sugar. Severe low blood sugar with symptoms like coma or seizures is rare, but can happen.
