Indications

It is used to treat the following conditions:

  • Acute and chronic psychosis
  • Mania
  • Schizophrenia

Pharmacology

It is a selective monoaminergic antagonist that primarily targets serotoninergic 5-HT2 and dopaminergic D2 receptors. It also interacts with alpha1 adrenergic receptors and, to a lesser extent, with H1 histamine and alpha2 adrenergic receptors. Unlike many other drugs, it does not affect cholinergic receptors. As a strong D2 antagonist, it improves the positive symptoms of schizophrenia while causing less motor activity depression. By balancing central serotonin and dopamine antagonism, it reduces the risk of side effects and helps manage both negative and emotional symptoms of schizophrenia.

Dosage & Administration

Psychoses:
Start with 2 mg on the first day (split into 1-2 doses), then 4 mg on the second day (split into 1-2 doses). The typical dose is 4-6 mg daily, but doses above 10 mg per day should only be used if the benefit is considered greater than the risk (maximum dose is 16 mg per day).
For elderly patients or those with liver or kidney problems, start with 1 mg daily, increasing by 1-2 mg twice daily as needed.
Children under 15 should not use this medication.

Mania:
Start with 2 mg once daily, increasing by 1 mg daily if needed. The typical dose range is 1-6 mg daily.
Elderly patients or those with liver or kidney problems should start with 1 mg daily in two doses, increasing by 1-2 mg twice daily.

Schizophrenia:
Initially, take 1 mg twice a day, increasing by 1 mg every two days until 3 mg twice a day is reached by the third day. Dosage adjustments should happen no more frequently than once a week, with small changes of 1-2 mg at a time.

Interaction

It may reduce the effects of levodopa and dopamine agonists. Long-term use with Carbamazepine decreases its levels in the blood, and using it with Clozapine may also lower its clearance. It can also increase the effectiveness of certain blood pressure medications.

It should not be used by people who are allergic to it.

Pregnancy & Lactation

Although no direct harm to reproduction was found in animal studies, some indirect effects related to prolactin and the central nervous system were observed. No birth defects were seen in any studies. The safety of using it during human pregnancy is not established. It is known to pass into animal and human breast milk, so women using it should avoid breastfeeding.

Precautions & Warnings

Special care should be taken if the person has heart problems, is elderly, has liver or kidney problems, epilepsy, Parkinson’s disease, or is pregnant. If symptoms of a movement disorder (tardive dyskinesia) occur, discontinue use.
