Indications

Ritopar 10 is used to treat uncomplicated premature labor.

Pharmacology

Ritopar 10 is a selective β2-adrenoceptor agonist that mainly affects the uterus, causing Ritopar 10 to relax. This reduces the intensity and frequency of contractions. Ritopar 10 also increases heart rate and lowers diastolic blood pressure. Ritopar 10 may cause bronchial relaxation, but this is not clinically significant in its use.

Dosage & Administration

By intravenous infusion: Start with 50 mcg/minute, gradually increasing by 50 mcg/min every 10 minutes until contractions stop or the maternal heart rate reaches 140 beats per minute. Continue for 12–48 hours after contractions stop (usual rate is 150–350 mcg/min). Maximum rate is 350 mcg/min.

Intramuscular injection: 10 mg every 3–8 hours for 12–48 hours after contractions stop; then by mouth.

Oral maintenance: Take one 10 mg tablet about 30 minutes before ending intravenous therapy, then repeat every 2 hours for 24 hours, followed by 10–20 mg every 4–6 hours (maximum 120 mg daily).

Interaction

The risk of low potassium (hypokalemia) increases when ritodrine is taken with high doses of corticosteroids, diuretics (such as acetazolamide, loop diuretics, and thiazides), or theophylline. β-adrenergic blockers should be avoided as they can interfere with its effect. Anesthetics may increase the risk of low blood pressure.

Contraindications

Ritopar 10 is contraindicated in patients who are allergic to the active ingredient or any component of this medication. Ritopar 10 should not be used in patients with cardiac disease, those at risk for heart issues, or with conditions like antepartum hemorrhage, intrauterine infection, fetal death, placenta previa, abruptio placenta, threatened miscarriage, or severe preeclampsia.

Side Effects

Common side effects include nausea, vomiting, flushing, sweating, tremors, low potassium, rapid heart rate, palpitations, low blood pressure, uterine bleeding, pulmonary edema, chest pain, irregular heartbeat, and swelling of salivary glands. Long-term use may cause low white blood cell count, liver problems, and hepatitis.

Pregnancy & Lactation

Ritopar 10 is classified as pregnancy category B. Ritopar 10 should not be used before 20 weeks of pregnancy as there are no controlled studies. After the 20th week, Ritopar 10 has not been shown to increase the risk of birth defects. However, Ritopar 10 should only be used if the benefits to the mother outweigh the risks to the baby. Newborns of mothers treated with ritodrine may experience symptoms like low blood sugar and low blood calcium.

Precautions & Warnings

Use Ritopar 10 cautiously in patients with heart disease, high blood pressure, thyroid problems, low potassium, diabetes, or mild preeclampsia. Blood pressure and pulse should be monitored, and fluid intake should be controlled during treatment.

Overdose Effects

Symptoms of overdose include rapid heart rate, palpitations, heart arrhythmias, low blood pressure, anxiety, tremors, nausea, and vomiting. If an overdose occurs, stomach pumping or induced vomiting should be performed, followed by activated charcoal. If overdose symptoms occur with injection, ritodrine should be stopped, and a β-blocking agent may be used as an antidote.
