Indications
Sacutan 50 is used to reduce the risk of death and hospitalization from heart failure in patients with chronic heart failure and a reduced heart pumping ability. Sacutan 50 is also used to treat heart failure in children aged one year and older. This medicine is usually taken alongside other heart failure treatments, replacing an ACE inhibitor or another angiotensin receptor blocker (ARB).
Pharmacology
Sacutan 50 contains two main ingredients: sacubitril, which is a neprilysin inhibitor, and valsartan, which is an angiotensin receptor blocker. Sacubitril works by increasing the levels of beneficial peptides, like natriuretic peptides, while valsartan blocks the harmful effects of angiotensin II. Together, they help improve heart and kidney function in heart failure patients.
Dosage & Administration
Adult Heart Failure: The usual starting dose is 49/51 mg twice a day. After 2 to 4 weeks, if tolerated, the dose may be increased to 97/103 mg twice a day.
For certain patients: If you have severe kidney or liver problems, or if you’re not currently taking an ACE inhibitor or ARB, start with a lower dose of 24/26 mg twice a day.
Pediatric Heart Failure: For children aged one year and older, the dose depends on the child’s weight and is adjusted every 2 weeks as needed.
Recommended Dose Titration for Children:
- For children under 40 kg: Start with 1.6 mg/kg, increase to 2.3 mg/kg, and then 3.1 mg/kg.
- For children 40-50 kg: Start with 24/26 mg, then increase to 49/51 mg.
- For children over 50 kg: Start with 49/51 mg, then increase to 72/78 mg, and finally 97/103 mg.
Interaction
Do not use Sacutan 50 with ACE inhibitors, aliskiren in patients with diabetes, or other ARBs. Potassium-sparing diuretics may increase potassium levels. Taking NSAIDs with Sacutan 50 may increase the risk of kidney problems. Be careful if you take lithium, as Sacutan 50 may increase the risk of toxicity.
Contraindications
Sacutan 50 is not recommended for people who are allergic to any of its ingredients, have had angioedema from ACE inhibitors or ARBs, or are using aliskiren if they have diabetes. Also, Sacutan 50 should not be used within 36 hours of switching from or to an ACE inhibitor.
Side Effects
Common side effects include swelling (angioedema), low blood pressure (hypotension), kidney problems, high potassium levels (hyperkalemia), cough, and dizziness.
Pregnancy & Lactation
Safety has not been established for use in children under 1 year old. In pregnant or breastfeeding women, consult a doctor before use. Sacutan 50 is not recommended during the first trimester of pregnancy unless there is a confirmed iron deficiency.
Precautions & Warnings
This medicine may cause angioedema, especially in people with a history of this condition. Sacutan 50 can lower blood pressure, so monitor kidney function and potassium levels regularly. In people with kidney or liver problems, dose adjustments may be necessary.
Overdose Effects
Overdose may cause low blood pressure. Treatment should focus on managing symptoms. This medicine is unlikely to be removed by hemodialysis due to its high protein binding.
