Indications

Salflu HFA 250 is used for the regular treatment of asthma in patients who need both a long-acting beta-agonist and an inhaled corticosteroid. Salflu HFA 250 is for patients who are not controlled by inhaled corticosteroids alone or who are already controlled on both inhaled corticosteroid and long-acting beta-agonist.

Pharmacology

Salflu HFA 250 is a combination of two drugs: Salmeterol and Fluticasone Propionate. Salmeterol is a long-acting beta-2 agonist that helps treat asthma and other lung problems. Fluticasone Propionate is a corticosteroid that reduces inflammation in the lungs. Salmeterol helps prevent symptoms, while Fluticasone Propionate improves lung function and reduces flare-ups. Together, they offer a convenient treatment for people who need both types of medication.

Salmeterol works by binding to the receptor in the lungs to help open airways. Fluticasone Propionate works by reducing inflammation in the lungs, making Salflu HFA 250 easier to breathe.

Dosage & Administration

Inhaler:

Adults and adolescents (12 years and older): 2 puffs of 25 µg Salmeterol and 50 µg Fluticasone Propionate twice daily, or 2 puffs of 25 µg Salmeterol and 125 µg Fluticasone Propionate twice daily, or 2 puffs of 25 µg Salmeterol and 250 µg Fluticasone Propionate twice daily.

Children (4-12 years): 2 puffs of 25 µg Salmeterol and 50 µg Fluticasone Propionate twice daily.

Inhalation capsule (for asthma):

Adults and adolescents (12 years and older): Salmeterol 50 µg & Fluticasone 100 µg or Salmeterol 50 µg & Fluticasone 250 µg twice daily (morning and evening, about 12 hours apart). The maximum dosage is Salmeterol 50 µg & Fluticasone 500 µg twice daily.

Pediatric patients (4-11 years): Salmeterol 50 µg & Fluticasone 100 µg twice daily (morning and evening, about 12 hours apart).

Inhalation capsule (for COPD): Salmeterol 50 µg & Fluticasone 250 µg twice daily (morning and evening, about 12 hours apart).

Maxhaler (for asthma):

Adults and adolescents (12 years and older): One inhalation twice daily for 50/100, 50/250, or 50/500 inhalers.

Children (4 years and older): One inhalation twice daily for 50/100 inhaler. The maximum dose for children is 100 mcg twice daily.

Interaction

Salflu HFA 250 is important to avoid using non-selective or selective beta-blockers with this medicine, unless necessary. Interactions with strong CYP3A4 inhibitors (like ketoconazole or ritonavir) can increase the amount of Fluticasone Propionate in the body, so caution is needed.

Contraindications

Salflu HFA 250 is not recommended for patients who are allergic to Salmeterol, Fluticasone Propionate, or any of its ingredients. Salflu HFA 250 should not be used as the primary treatment for severe asthma attacks or other sudden asthma issues that require immediate treatment.

Side Effects

Some common side effects include shaking, palpitations, headaches, and throat irritation. There may also be rare side effects like muscle cramps, skin rashes, and swelling. Hoarseness and mouth infections (thrush) are more likely with Fluticasone Propionate. These can be reduced by gargling with water after using the inhaler.

Pregnancy & Lactation

If pregnant or breastfeeding, consult a doctor before using this medicine. Salflu HFA 250 should only be used if the benefit to the mother outweighs any potential risks to the baby. There is limited information about its safety during pregnancy or breastfeeding.

Precautions & Warnings

Salflu HFA 250 should not be used during an asthma attack or if asthma is worsening. Salflu HFA 250 is not meant for quick relief. Patients should always have their fast-acting inhalers on hand. Serious asthma-related problems and worsened symptoms may occur during treatment, and medical help should be sought if symptoms don’t improve. Paradoxical bronchospasm (sudden difficulty breathing) may occur and should be treated immediately with a fast-acting inhaler.

Overdose Effects

Symptoms of overdose may include seizures, rapid heart rate, muscle cramps, dry mouth, dizziness, and confusion. If overdosed, treatment may involve stopping the medicine and using beta-blockers to manage the symptoms. Excessive use of Fluticasone Propionate may cause temporary suppression of adrenal function, but this typically recovers within a few days.

Store below 30°C. Keep away from sunlight, heat, and moisture. Do not puncture, break, or burn the canister. Keep out of reach of children.
