Indications
Stemetil is indicated to control severe nausea and vomiting caused by radiation therapy, chemotherapy, surgery, and other conditions. Stemetil also helps relieve nausea, vomiting, and dizziness (vertigo) associated with Meniere’s disease and other inner ear disorders. Additionally, Stemetil is used in the treatment of psychotic illnesses like schizophrenia, acute mania, and short-term treatment of generalized non-psychotic anxiety.
Pharmacology
Stemetil works as a dopamine and histamine antagonist. The anti-nausea effect occurs due to blocking histamine H1 and dopamine D2 receptors in the brain’s vomiting centers. Stemetil also has a mild anticholinergic effect and helps prevent acid reflux by improving the tone of the lower esophageal sphincter.
Dosage & Administration
Antiemetic:
Children: (not recommended for children under 2 years or weighing less than 10 kg)
10-14 kg: 2.5 mg every 12-24 hours as needed (max: 7.5 mg/day)
15-18 kg: 2.5 mg every 8-12 hours as needed (max: 10 mg/day)
19-39 kg: 2.5 mg every 8 hours or 5 mg every 12 hours (max: 15 mg/day)
Adults: 5-10 mg 3-4 times a day (max: 40 mg/day)
Antipsychotic:
Children: (not recommended for children under 2 years or weighing less than 10 kg)
2-12 years: 2.5 mg 2-3 times a day (increase up to max: 20 mg for 2-5 years, 25 mg for 6-12 years)
Adults: 5-10 mg 3-4 times a day (up to 150 mg/day for severe cases)
Nonpsychotic anxiety:
Adults: 15-20 mg/day in divided doses (max: 20 mg/day or 12 weeks of use)
Elderly: Start with 2.5-5 mg 1-2 times/day and gradually increase dose, max: 75 mg/day
Stemetil can be taken with or without food.
Interaction
Taking alcohol, barbiturates, or sedatives with this medicine may increase drowsiness. Some drugs like antacids, antiparkinson’s drugs, and lithium can affect the absorption of this medication. Stemetil may also interact with medications like propranolol and phenobarbital.
Contraindications
Stemetil is contraindicated in individuals who are allergic to Stemetil or any of its components. Stemetil should not be used in severe CNS depression, coma, or in children under 2 years or weighing less than 10 kg.
Side Effects
Side effects may include drowsiness, muscle spasms in the jaw, neck, or back, involuntary tongue or mouth movements, difficulty speaking or swallowing, restlessness, tremors, shuffling walk, skin rash, or yellowing of the skin and eyes.
Pregnancy & Lactation
No harmful effects have been reported during pregnancy or breastfeeding. However, Stemetil is recommended to consult a doctor before use.
Precautions & Warnings
Be cautious when performing activities requiring alertness, such as driving. Alcohol may cause extreme drowsiness. Stemetil may also increase sensitivity to sunlight, so avoid prolonged sun exposure and wear sunscreen and protective clothing. Stemetil can reduce sweating, making heatstroke more likely in hot conditions.
Overdose Effects
Overdose symptoms include deep sleep, coma, abnormal muscle movements, and low blood pressure.
