Indications
Suxonium is used as a muscle relaxant in anesthesia to make Suxonium easier to insert a tube into the windpipe (endotracheal intubation), help with mechanical ventilation, and assist in various surgical and obstetric procedures.
Pharmacology
Suxonium works as a muscle relaxant by acting on the neuromuscular junction. Suxonium mimics acetylcholine, a chemical that helps muscles contract. When Suxonium binds to muscle receptors, Suxonium causes a temporary muscle twitch (fasciculation). Unlike acetylcholine, which is broken down quickly by acetylcholinesterase, Suxonium is broken down slowly by another enzyme. This causes the muscle to remain relaxed for a longer time, as the muscle cannot contract properly after the initial twitch.
Dosage & Administration
Adults: Suxonium is usually given as an injection, either intravenously or intramuscularly. For endotracheal intubation, a dose of 1 mg/kg is given intravenously, which causes muscle relaxation in about 30-60 seconds and lasts for 2-6 minutes. For longer procedures, Suxonium may be given as an intravenous infusion. The total dose should not exceed 500 mg per hour.
Children: Infants and young children may require a lower dose than adults. The recommended dose for infants is 2 mg/kg intravenously, and for older children, Suxonium is 1 mg/kg. The maximum dose for infants is 150 mg.
Interaction
Some drugs may reduce the effectiveness of plasma cholinesterase and prolong the effects of this medication. These include certain anticholinesterase drugs, chemotherapy drugs, psychiatric medications, and anesthetics like ketamine, morphine, and pethidine.
Contraindications
Suxonium should not be given to patients who are not fully anesthetized or those who are allergic to the drug. Suxonium is also contraindicated in people with certain genetic conditions that affect plasma cholinesterase activity. Suxonium should not be used in patients with severe burns, major trauma recovery, muscle wasting conditions, or pre-existing high potassium levels. Suxonium can cause a slight rise in eye pressure, so Suxonium should not be used for open eye injuries.
Side Effects
Possible side effects include changes in heart rate (bradycardia or tachycardia), blood pressure changes, breathing problems, muscle pain, fever, rash, and increased eye pressure.
Pregnancy & Lactation
Suxonium does not easily cross the placenta, but should only be used during pregnancy if the benefits outweigh the risks. Suxonium should not be used during breastfeeding unless necessary.
Precautions & Warnings
Suxonium should be administered under the supervision of an experienced anesthetist. Elderly patients may be more prone to heart problems, especially if they are also taking heart medications.
