Indications

Syndopa CR 125 is used to treat idiopathic Parkinson’s disease, especially to reduce off-periods in patients who have already been treated with levodopa or levodopa with decarboxylase inhibitors and have experienced motor fluctuations.

Dosage

For patients switching from conventional levodopa: The dosage of Levodopa-Carbidopa prolonged-release tablet should be started at a level that provides about 10% more levodopa daily. The doses can be adjusted based on clinical response. If a patient is taking higher doses, the dosing interval should be 4-12 hours, with a 30-50% longer gap between doses. Dosage can be adjusted as needed based on individual response.

For example:

  • Daily dosage of 300-400 mg of conventional levodopa = 400 mg Levodopa-Carbidopa prolonged-release tablet, 1 tablet 2x daily.
  • Daily dosage of 500-600 mg of conventional levodopa = 600 mg Levodopa-Carbidopa prolonged-release tablet, 1 tablet 3x daily.
  • And so on…

Interaction

Syndopa CR 125 is important to be cautious when using Levodopa-Carbidopa with the following medications:

  • Antihypertensive agents: Some blood pressure medications may require dosage adjustments when taken with this tablet.
  • Antidepressants: Rare reactions like high blood pressure and movement issues may occur with tricyclic antidepressants.
  • Anticholinergics: These may affect how well the medicine works.
  • Iron supplements: Iron can reduce the effectiveness of levodopa.
  • Other drugs: Certain drugs like phenothiazines, risperidone, and isoniazid may reduce levodopa’s effects. Also, taking the tablet with food, especially high-protein meals, might reduce its effectiveness.

Contraindications

Syndopa CR 125 is not recommended for people who are allergic to any of its components or have narrow-angle glaucoma. Syndopa CR 125 should also not be used in patients with undiagnosed skin lesions or a history of melanoma.

Side Effects

The most common side effect is dyskinesia (uncontrolled movements). Other possible side effects include nausea, dizziness, confusion, dry mouth, difficulty sleeping, depression, chest pain, and digestive issues like constipation and diarrhea. Rarely, more severe side effects like hallucinations or heart problems may occur. Patients should report any unusual symptoms to their doctor.

Pregnancy & Lactation

Syndopa CR 125 is not known if this medication is harmful during pregnancy or while breastfeeding. Levodopa has been found in breast milk, so Syndopa CR 125 should not be used by pregnant women or nursing mothers unless directed by a doctor.

Precautions & Warnings

Syndopa CR 125 should be used cautiously in patients with heart disease, asthma, liver, or kidney problems, or a history of ulcers or seizures. Patients should also be monitored for sudden sleep episodes, movement disorders, and mental health changes. Dosage may need adjustment if side effects occur.
