Indications
Syntocin is used for inducing labor and stimulating uterine contractions. Syntocin is also used after delivery to prevent or treat secondary bleeding and to help with adherent placenta. Syntocin can stimulate lactation, but only if advised by a doctor. Additionally, Syntocin is used in cases of incomplete abortion to speed up the process and prevent further bleeding.
Pharmacology
Syntocin works by increasing the calcium levels inside cells, which helps the uterus contract. The muscles of the uterus have special receptors for Syntocin, which become more active as pregnancy progresses, especially during labor.
Dosage & Administration
Induction of labor and stimulation of uterine contractions: Syntocin should be given through an intravenous drip, starting with 1-5 IU added to 500 ml of fluid. The dose can be increased every 30-45 minutes until the right level of contraction is reached, usually below 10 mU per minute.
Post delivery: For preventing or treating bleeding or placenta issues, an intramuscular injection of 2-5 IU is recommended, or Syntocin can be given as an intravenous infusion.
To stimulate lactation: An intramuscular injection of 2 IU is used, but only with a doctor’s advice.
For incomplete abortion: An intramuscular injection of 2-5 IU every 30-60 minutes can help speed up the process, or Syntocin can be given through an intravenous infusion in appropriate doses.
Interaction
Severe high blood pressure can occur if Syntocin is given after a vasoconstrictor with anesthesia. Anesthesia with cyclopropane can change its effects and cause low blood pressure. Using Syntocin with this type of anesthesia may also cause heart rate issues in the mother.
Contraindications
Syntocin should not be used in cases of a large baby, abnormal baby position, or abnormal skull position. Syntocin is also contraindicated in cases of over-stretched uterus, severe preeclampsia, hyperactive uterus, fetal distress when delivery is not immediate, cord prolapse, placenta previa, or vasa previa.
Side Effects
Possible side effects include uterine overactivity, spasm, or even rupture. There may also be increased bleeding or low levels of fibrinogen in the blood. Serious water intoxication can happen, leading to convulsions and coma. Other side effects might include nausea, vomiting, hemorrhage, heart arrhythmias, or allergic reactions. Rarely, jaundice in newborns has been reported after use.
Pregnancy & Lactation
Syntocin is categorized as pregnancy category C. Syntocin is not known if Syntocin passes into breast milk, so Syntocin should only be used during pregnancy or breastfeeding if advised by a doctor.
Precautions & Warnings
Syntocin should be used carefully, especially in patients with previous uterine scarring. The right dose must be found for each person to ensure safe labor induction. Only use Syntocin with proper medical supervision. The fetal heart rate and mother’s blood pressure should be closely monitored while using Syntocin.
Overdose Effects
Too much oxytocin can cause very strong contractions, which may lead to a slow heart rate in the baby, heart problems, severe tissue tears, or even uterine rupture. If an overdose occurs, the infusion should be stopped immediately and the necessary steps to stop contractions should be taken.
