Indications

T-H is used to treat thiamine deficiency conditions like Beriberi, Wernicke’s encephalopathy, pregnancy-related neuritis, and pellagra. T-H is also used when there is low thiamine intake or when the body cannot absorb enough thiamine, such as in alcohol consumption or during pregnancy or a diet high in carbohydrates.

Pharmacology

Thiamine acts as a coenzyme for a process in the body called decarboxylation of α-ketoglutaric acid. A lack of thiamine affects the nervous system, digestive system, and heart. Thiamine is essential for healthy growth in infants and children. The body doesn’t store thiamine, so T-H needs to be replenished daily. Without enough thiamine, a person may feel tired, lose appetite, have digestive problems, a fast heart rate, irritability, and nerve issues. Thiamine deficiency can cause Beriberi, which is common in alcoholics, pregnant women with poor diets, and people with digestive issues or liver disease.

Dosage & Administration

Prophylaxis: 3 to 10 mg daily.
Mild chronic deficiency: 10 to 25 mg daily.
Severe deficiency: 200 to 300 mg daily.

Interaction

No harmful drug interactions have been reported. Vitamin B1 works well with other B vitamins and its risk of causing side effects is low.

Contraindications

There are no absolute contraindications, but there is an increased risk of allergic reactions, such as anaphylaxis, with repeated parenteral (injection) use. Mild reactions, like sneezing or mild asthma, may signal a risk for more serious reactions. T-H is advisable to start with a lower dose if a second dose is needed. Vitamin B1 injections should not be given intravenously unless the patient is in a coma. After correcting thiamine deficiency, parenteral doses are not necessary.

Side Effects

When taken orally, Vitamin B1 doesn’t usually cause side effects. However, rare cases of anaphylactic reactions have been reported after high doses (400 mg) of intravenous thiamine, particularly in sensitive children. The risk of such reactions increases with repeated injections. Mild soreness may occur at the injection site.

Pregnancy & Lactation

T-H is safe to use during pregnancy, breastfeeding, and in neonates, children, and elderly patients.
