Indications

T-Zol is used to treat trichomoniasis caused by the Trichomonas vaginalis parasite. Both the patient and their sexual partner should be treated to avoid re-infection. T-Zol is also used to treat giardiasis, an infection caused by Giardia duodenalis, in adults and children over three years old. T-Zol helps with intestinal amebiasis and amebic liver abscess caused by Entamoeba histolytica in both adults and children older than three years. However, T-Zol is not used for asymptomatic cyst passage. T-Zol also treats bacterial vaginosis in non-pregnant women, but other causes of vulvovaginitis must be ruled out first. T-Zol should only be used to treat infections confirmed or strongly suspected to be caused by susceptible bacteria to prevent drug resistance.

Pharmacology

T-Zol is a drug similar to metronidazole and works against protozoa (such as Trichomonas vaginalis, Entamoeba histolytica, and Giardia lamblia) and certain types of bacteria. T-Zol interferes with the DNA of these microorganisms, preventing them from multiplying and causing infection.

Dosage & Administration

Prevention of Postoperative Infections: Adults: One 2g dose taken 12 hours before surgery.
Children under 12: No dosage recommendation available for children under 12 for preventing anaerobic infections.
Trichomoniasis: One 2g dose with food. Both the patient and their partner should be treated at the same time.
Giardiasis: Adults: One 2g dose with food.
Children over three years: One dose of 50mg/kg (up to 2g) with food.
Amebiasis: Adults: 2g per day for three days with food.
Children over three years: 50mg/kg per day (up to 2g) for three days with food.
Amebic Liver Abscess: Adults: 2g per day for 3-5 days with food.
Children over three years: 50mg/kg per day (up to 2g) for 3-5 days with food.
Bacterial Vaginosis: Non-pregnant women: 2g once daily for 2 days with food or 1g once daily for 5 days with food.

Interaction

T-Zol can interact with several drugs. Avoid alcohol and disulfiram during treatment and for three days afterward as they can cause nausea, vomiting, and headaches. T-Zol may also increase the effects of anticoagulants like warfarin, so their dosage may need adjusting. Drugs like cyclosporine, lithium, and tacrolimus may have higher levels, increasing the risk of toxicity. Tinidazole may also interact with other medications that affect the liver, such as phenytoin, and alter its effectiveness.

Contraindications

T-Zol should not be used in patients with blood disorders or neurological problems. Avoid T-Zol if you are allergic to the drug.

Side Effects

Most side effects are mild and go away on their own. These can include nausea, vomiting, loss of appetite, diarrhea, and a metallic taste. In rare cases, skin reactions like rash and swelling may occur. T-Zol may also cause a decrease in white blood cells, leading to infections, tiredness, headache, and changes in urine color.

Pregnancy & Lactation

T-Zol is not recommended during the first trimester of pregnancy. In later stages, T-Zol should only be used if the potential benefits outweigh the risks to the baby. T-Zol passes into breast milk, so breastfeeding should be stopped during treatment and for three days after the last dose.

Precautions & Warnings

T-Zol can cause neurological symptoms like dizziness and lack of coordination. If these symptoms occur, treatment should be stopped. T-Zol should not be used during the first trimester of pregnancy, and its use in the later stages should only happen if necessary. T-Zol can be found in breast milk, so breastfeeding should be stopped during treatment and for three days afterward.
