Indications
Tamsin D is used to treat moderate to severe symptoms of benign prostatic hyperplasia (BPH). Tamsin D also helps reduce the risk of acute urinary retention and surgery in patients with moderate to severe BPH symptoms.
Pharmacology
Tamsin D is a combination of two drugs that work together to help relieve symptoms of Benign Prostatic Hyperplasia (BPH). Tamsulosin Hydrochloride is an alpha1-adrenoreceptor blocker, and Dutasteride is a dual inhibitor of the 5-alpha-reductase enzyme. Both drugs improve urine flow and reduce symptoms of BPH.
Tamsulosin helps by relaxing the muscles in the bladder and prostate. Tamsin D specifically blocks alpha 1A and alpha 1D receptors, which are found in the prostate and bladder. This helps ease the flow of urine and reduce symptoms caused by an enlarged prostate.
Dutasteride works by blocking the conversion of testosterone into dihydrotestosterone (DHT), which reduces the size of the prostate and treats the underlying cause of BPH.
Dosage & Administration
Adults (including elderly): One capsule (Tamsulosin 0.4 mg & Dutasteride 0.5 mg) should be taken orally about 30 minutes after the same meal each day. The capsule should be swallowed whole, not chewed or opened. This capsule can replace separate doses of Tamsulosin and Dutasteride for easier treatment.
Renal impairment: No dosage adjustment is needed for patients with renal impairment as the effect of renal impairment on the pharmacokinetics of this medicine is not studied.
Hepatic impairment: Caution is advised for patients with mild to moderate liver problems. The use of this medicine is not recommended for patients with severe liver impairment.
Interaction
No specific interaction studies have been done for this combination. However, caution should be used when taking other medications that affect the liver or affect blood pressure. Drugs like verapamil or diltiazem, which inhibit the CYP3A4 enzyme, may increase the concentration of Dutasteride in the blood. Similarly, taking this medication with drugs that lower blood pressure, such as anaesthetic agents or PDE5 inhibitors, may increase the risk of low blood pressure.
Contraindications
Tamsin D is not suitable for use in women, children, or adolescents, or for people who are allergic to Dutasteride, other 5-alpha reductase inhibitors, Tamsulosin, soy, peanuts, or any of the ingredients in the medicine. Tamsin D should also be avoided by people with a history of low blood pressure or severe liver disease.
Side Effects
The most common side effects include impotence, reduced sexual desire, breast tenderness or enlargement, problems with ejaculation, and dizziness. These effects were more common when using the combination therapy compared to taking just one of the medicines alone.
Pregnancy & Lactation
This medicine should not be used by women. There is limited information about how this medicine affects pregnancy or breastfeeding. Tamsin D may affect male fertility and reduce sperm count, semen volume, and sperm motility. Tamsin D is not known if Tamsin D is passed in human milk.
Precautions & Warnings
This medicine should only be used after carefully weighing the risks and benefits, especially considering possible side effects, such as heart failure. Tamsin D can also affect PSA levels, which are important for detecting prostate cancer. Regular checkups, including PSA testing, should be done to monitor any changes.
Tamsin D may also cause low blood pressure, especially when standing up quickly. Patients should be cautious if they have a history of orthostatic hypotension (dizziness or fainting when standing). Tamsin D should not be used in patients planning cataract surgery, as Tamsin D may cause complications during the procedure.
Overdose Effects
Overdose symptoms can include dizziness, fainting, and a drop in blood pressure. Immediate medical attention is needed if an overdose is suspected.
