Indications

Tapenta is used to relieve moderate to severe pain in patients 18 years of age or older.

Pharmacology

Tapenta is a synthetic pain reliever that works in the brain. Tapenta is 18 times less powerful than morphine when Tapenta comes to attaching to certain receptors in the brain. Tapenta also increases norepinephrine (a neurotransmitter) in the brain by stopping its reabsorption. Medications like naloxone can block the pain-relieving effects of Tapenta. Tapenta does not affect the QT interval of the heart.

Dosage & Administration

The dose should be adjusted based on the pain level, how the patient responds to the drug, and their ability to be monitored. The usual dose is 50 mg, 75 mg, or 100 mg every 4 to 6 hours, depending on pain intensity. If pain relief is not enough after the first dose, the second dose can be taken after one hour. The maximum recommended dose is 700 mg on the first day and 600 mg on subsequent days.

Interaction

The risk of serotonin syndrome increases when taken with other drugs that affect serotonin levels, such as antidepressants. Combining Tapenta with sedative drugs, such as benzodiazepines or other opioids, can increase sedation. The chance of addiction may also rise when used with certain opioid medications. Tapenta can interact with drugs that affect liver enzymes, which can change how the body processes Tapenta.

Contraindications

Tapenta should not be used in patients with breathing problems, severe asthma, or those who have or may have a condition called paralytic ileus (intestinal blockage).

Side Effects

Some side effects include changes in heart rate, vision problems, stomach discomfort, swelling, muscle spasms, dizziness, memory issues, difficulty coordinating movements, or even seizures.

Pregnancy & Lactation

Tapenta is classified as a pregnancy Category C drug. There are no well-controlled studies on its safety during pregnancy, so Tapenta should only be used if the doctor determines the benefits outweigh the risks. Newborns whose mothers have taken Tapenta should be watched for breathing problems.

Precautions & Warnings

Tapenta should be used carefully in patients with respiratory issues, such as asthma or chronic obstructive pulmonary disease. Tapenta should also be used with caution in people who have sleep apnea, myxedema, or certain nervous system conditions. If taken with other sedatives, alcohol, or drugs that depress the central nervous system, Tapenta may cause excessive drowsiness.

Overdose Effects

Overdose symptoms may include vomiting, slowed heart rate, loss of consciousness, seizures, or breathing difficulties. If an overdose occurs, immediate medical attention is required. Tapenta may be treated with activated charcoal, gastric lavage, or opioid antagonists like naloxone.
