Indications
Temcard-A 5/40 is used to treat high blood pressure, either alone or with other blood pressure-lowering medicines. Temcard-A 5/40 may also be used as the first treatment for patients who are likely to need more than one medicine to control their blood pressure.
Pharmacology
Temcard-A 5/40 is a combination of Telmisartan and Amlodipine. Telmisartan is a medicine that blocks the effects of a hormone that raises blood pressure. Temcard-A 5/40 helps to relax and widen blood vessels, reducing blood pressure without affecting heart rate. Amlodipine is a calcium channel blocker that works by relaxing the blood vessels and lowering blood pressure by reducing the resistance in the blood vessels.
Dosage & Administration
Initial Therapy: Temcard-A 5/40 can be started if Temcard-A 5/40 is unlikely that blood pressure control will be achieved with just one medicine. The usual starting dose is 40/5 mg once daily. If more reduction is needed, the dose may be increased to 80/5 mg daily. This is not recommended for patients older than 75 years or those with liver problems.
Additional Therapy: For patients not controlled with Amlodipine or Telmisartan alone, Temcard-A 5/40 can be used. If a patient experiences side effects like swelling with Amlodipine 10 mg, the dose can be switched to this 40/5 mg tablet once daily.
Replacement Therapy: Patients taking separate tablets of Amlodipine and Telmisartan can switch to this tablet with the same doses once daily. The dosage may be adjusted after 2 weeks, with a maximum dose of 80/10 mg daily.
Interaction
Temcard-A 5/40 does not have major interactions with acetaminophen, Amlodipine, glyburide, simvastatin, hydrochlorothiazide, warfarin, or ibuprofen. Temcard-A 5/40 does not affect the metabolism of most drugs, except for possibly some effects on CYP2C19. Amlodipine has been safely used with other medications like diuretics, beta-blockers, ACE inhibitors, and others without significant issues.
Contraindications
Temcard-A 5/40 should not be used by patients who are allergic to any of its ingredients or are pregnant or breastfeeding. Temcard-A 5/40 is also not for patients with certain liver or heart conditions, like severe liver disease or heart failure.
Side Effects
Possible side effects include dizziness, swelling, headache, nausea, and fatigue. Some people may also experience chest pain, difficulty breathing, or problems with their heart rate.
Pregnancy & Lactation
Temcard-A 5/40 is not recommended during pregnancy, especially in the first trimester, and should be avoided unless clearly necessary. Temcard-A 5/40 is unknown if Temcard-A 5/40 passes into breast milk, so nursing mothers should consult their doctor before use.
Precautions & Warnings
Temcard-A 5/40 should be used with caution in patients who have low blood pressure, kidney issues, or heart problems. Temcard-A 5/40 is important to monitor for symptoms of low blood pressure and avoid using Temcard-A 5/40 with other certain blood pressure medications like ACE inhibitors.
Overdose Effects
In case of overdose, symptoms may include low blood pressure, dizziness, and fast heart rate. If this occurs, medical attention should be sought immediately. Temcard-A 5/40 is not removed by hemodialysis.
