Indications
Terbitac is used to treat fungal infections of the toenails or fingernails caused by dermatophytes (tinea unguium). Terbitac is also used to treat Tinea Capitis (scalp fungal infection). Terbitac is effective against skin infections caused by fungi such as Trichophyton, Microsporum, and Epidermophyton. Terbitac can treat yeast infections of the skin, particularly those caused by Candida species. Terbitac is also used for Pityriasis (tinea) versicolor, a skin condition caused by Malassezia furfur.
Pharmacology
Terbitac works by inhibiting the production of Ergosterol, a key part of fungal cell membranes, through blocking the Squalene Epoxidase enzyme. This causes fungal cells to die. Depending on the drug concentration and the fungal species, Terbitac may be fungicidal (kills fungi). Terbitac has been shown to be effective against several types of fungi, including Trichophyton species.
Dosage & Administration
Terbinafine tablet: For fingernail onychomycosis, take one 250 mg tablet daily for 6 weeks. For toenail onychomycosis, take one 250 mg tablet daily for 12 weeks. The best results are seen after a few months when the healthy nail grows out.
Terbinafine granules: For patients weighing less than 25 kg, take 125 mg daily for up to 6 weeks. For those weighing 25-35 kg, take 187.5 mg daily. For patients weighing over 35 kg, take 250 mg daily for up to 6 weeks.
Terbinafine cream: Apply once or twice daily. Clean and dry the affected area before use. Apply a thin layer of cream and rub in gently. Treatment durations vary based on infection type:
- Tinea corporis and cruris: 1-2 weeks
- Tinea pedis: 1 week
- Cutaneous candidiasis: 2 weeks
- Pityriasis versicolor: 2 weeks
Terbinafine 1% Spray: Apply once or twice daily. Clean and dry the affected areas before applying. Treatment durations:
- Tinea pedis: once daily for 1 week
- Tinea corporis/cruris: once daily for 1 week
- Pityriasis versicolor: twice daily for 1 week
Interaction
Terbitac may interfere with drugs metabolized by the CYP450 2D6 enzyme, including tricyclic antidepressants, SSRIs, beta-blockers, antiarrhythmic drugs, and MAO inhibitors. If taking such medications, careful monitoring and dose adjustments may be needed.
Contraindications
Terbitac should not be used by people who are allergic to terbinafine.
Side Effects
Side effects can include stomach issues (like diarrhea, indigestion, and stomach pain), liver problems, rashes, itching, taste changes, and other mild, short-term reactions. Rare but serious side effects may include liver failure, skin reactions, severe low white blood cell count, allergic reactions, and hair loss.
Pregnancy & Lactation
There are no well-controlled studies on its use during pregnancy, so Terbitac is not recommended during pregnancy unless necessary. Terbitac is also present in breast milk, so Terbitac should be avoided by nursing mothers.
Precautions & Warnings
Be cautious when using Terbitac in people with liver problems. Liver tests should be done before starting treatment, and treatment should stop if liver issues arise. Serious skin reactions and liver damage have occurred in some people using this medicine. Terbitac is for external use only and should not get into the eyes.
Overdose Effects
Overdose symptoms may include nausea, vomiting, stomach pain, dizziness, rash, frequent urination, and headaches. No serious side effects have been observed with doses up to 5 grams (20 times the usual daily dose).
