Indications

It is used as a long-term treatment to help relieve symptoms in adults with chronic obstructive pulmonary disease (COPD).

Pharmacology

This dry powder inhaler capsule contains a combination of Umeclidinium (an anticholinergic) and Vilanterol (a LABA) as an inhalation powder. When inhaled, the drug works by relaxing the smooth muscles in the airways and preventing the release of certain substances from mast cells that can cause inflammation.

Vilanterol, a beta-2 agonist, works by stimulating an enzyme called adenylate cyclase, which increases cyclic AMP levels in the cells. This results in the relaxation of the smooth muscles in the airways. Umeclidinium, a long-acting muscarinic antagonist, works by blocking muscarinic receptors in the airways, leading to airway dilation.

Dosage & Administration

This inhalation capsule should not be swallowed. Use it only with the inhaler device. Always remove the capsule from its blister pack just before using the inhaler. After each use, rinse your mouth with water to reduce the risk of oral thrush.

Adults (18 years and older): Take once a day at the same time every day to ensure it’s not used more than once in 24 hours.

Indicated dose for COPD maintenance therapy: One inhalation of the 25/62.5 mcg capsule once a day.

Limitations: This medication is not for use in treating sudden breathing difficulties or asthma attacks.

For children and teenagers: This drug is not recommended for use in children under 17 years old. Its safety and effectiveness have not been established in younger patients.

For elderly, renal, or moderate hepatic dysfunction patients: No dose adjustment is necessary.

Interaction

Be cautious when using this medication with strong CYP450 3A4 inhibitors (e.g., Ketoconazole). It may cause side effects related to the heart and blood vessels.

If using monoamine oxidase inhibitors (MAOIs) or tricyclic antidepressants, use with caution, as Vilanterol may increase effects on the vascular system.

Beta-blockers should be used carefully, as they can block the action of bronchodilators, potentially leading to breathing problems.

Be cautious when using diuretics, especially non-potassium-sparing ones, as they may cause changes in heart rhythm or low potassium levels that can worsen with beta-agonist medications.

Combining with other anticholinergic medications may increase the risk of side effects. Avoid using other inhalers that contain anticholinergics with this product.

Contraindications

It is contraindicated in patients who are allergic to milk proteins or any of its ingredients. This inhaler should not be used in asthma patients without a corticosteroid, as it may worsen symptoms.

Side Effects

The most common side effects in COPD patients include throat irritation, sinus infections, lower respiratory infections, constipation, diarrhea, hand pain, muscle cramps, neck pain, and chest pain.

Precautions & Warnings

Long-term use of beta-2 agonists like Vilanterol alone may increase the risk of severe asthma-related events. Do not start this medication during an acute COPD or asthma exacerbation. It should not be used as an emergency treatment for severe symptoms.

Do not use this medication with other long-acting beta-2 agonists unless advised by your doctor.
