Indications
Xaloplat is used to reduce the chance of recurring vulvovaginal candidiasis (RVVC) in females who have a history of RVVC and are not able to have children. If fungal culture tests are done before treatment, antifungal therapy can start before the results are ready. However, once the results are available, the treatment should be adjusted accordingly.
Pharmacology
Xaloplat is an antifungal medicine. The way Xaloplat works and the full effect over time are not fully understood. Xaloplat targets a fungal enzyme called 14α demethylase (CYP51), which is important for making a sterol called ergosterol. This sterol is needed for the structure and function of the fungal cell membrane. By blocking this enzyme, Xaloplat causes the accumulation of harmful sterols in the fungus, which ultimately kills the fungi. Xaloplat has a lower chance of affecting human enzymes compared to other similar drugs.
Dosage & Administration
Oteseconazole-only dosage: On Day 1: Take 600 mg as a single dose, then on Day 2: Take 450 mg, then starting on Day 14: Take 150 mg once a week for 11 weeks (for weeks 2 to 12).
Fluconazole/Oteseconazole dosage: On Day 1, Day 4, and Day 7: Take 150 mg of fluconazole, then from Day 14 to Day 20: Take 150 mg of oteseconazole once a day for 7 days, then from Day 28: Take 150 mg of oteseconazole once a week for 11 weeks (for weeks 4 to 14).
Take the medicine with food. Swallow the capsules whole. Do not chew, crush, dissolve, or open them.
Interaction
When taken with certain drugs (BCRP substrates), Xaloplat may increase the levels of these drugs in the body, which could cause more side effects. To reduce this risk, start with the lowest dose of the other medicine or reduce its dose and monitor for side effects.
Contraindications
Xaloplat is not recommended for:
- Females who can get pregnant
- Pregnant women
- Women who are breastfeeding
- Anyone with an allergy to oteseconazole
Side Effects
The most common side effects are headache and nausea.
Pregnancy & Lactation
Xaloplat is not recommended for use in women who are pregnant or breastfeeding. Based on animal studies, Xaloplat may harm the baby during pregnancy. The medicine stays in the body for a long time (about 690 days), so Xaloplat can affect a developing fetus or a breastfed baby. There are no data on how the drug affects milk or milk production in humans or animals.
Precautions & Warnings
Xaloplat should not be used in women who can get pregnant, or in pregnant or breastfeeding women, due to possible risks to the fetus or baby. Studies in animals have shown that the drug can cause serious eye problems in offspring. These problems occurred at doses lower than what is used in humans. Patients should be warned about these risks and advised against using the drug if they are pregnant or breastfeeding.
