Indications
Xamic IV/IM is used to prevent and treat bleeding in various situations such as lung and digestive bleeding, bleeding related to leukemia, cirrhosis, hemophilia, and other bleeding disorders. Xamic IV/IM is also used to stop bleeding during and after surgery, including heart, lung, and abdominal surgeries, and for shock after trauma. In urology, Xamic IV/IM helps prevent bleeding in prostate, bladder, and kidney surgeries. Xamic IV/IM can be used to treat bleeding after childbirth and certain conditions like excessive bleeding during menstruation. Xamic IV/IM is also useful in preventing bleeding during tonsil surgery and dental extractions. In cancer care, Xamic IV/IM helps reduce bleeding during surgeries and promotes the formation of a protective capsule around ovarian tumors.
Pharmacology
Xamic IV/IM is a medication made from tranexamic acid, which has a strong effect in stopping excessive bleeding. Xamic IV/IM works by blocking certain proteins that break down blood clots, making Xamic IV/IM 10 times stronger than traditional blood-clotting medications. The medicine has a low toxicity, is absorbed quickly when taken by mouth, and works for a longer time than other similar medications. Xamic IV/IM doesn’t interfere with normal clotting and does not cause blood clotting problems even after long use.
Dosage & Administration
Adults: The usual dose is 500-1000 mg, taken 3 times a day. For prevention, the recommended dose is 0.5-1 gm orally, or 500 mg via injection. For treating bleeding, the oral dose is increased to 1-3 gm in divided doses. In urgent cases, the first dose is given via slow injection, followed by oral doses.
Children: For prevention, give 5-10 mg per kg of body weight daily in divided doses. For treatment, the dose is increased to 10-20 mg per kg. In some cases, the injection can be given slowly by vein, followed by oral doses. In small babies, the medicine can be mixed with water and given orally.
Elderly: No dosage reduction is needed unless there is kidney failure.
Interaction
Xamic IV/IM is not compatible with penicillin-containing solutions. Thrombolytic drugs like Streptokinase and Urokinase can reduce its effect. Using Xamic IV/IM along with estrogen-containing medications, like birth control pills, may increase the risk of blood clots. Xamic IV/IM should not be mixed directly with whole blood during transfusions.
Contraindications
Xamic IV/IM should not be used by individuals with known allergies to the medication or those with blood clotting disorders, including deep vein thrombosis, pulmonary embolism, or severe kidney problems.
Side Effects
Xamic IV/IM is usually well tolerated, but some may experience mild side effects like fatigue, eye irritation, nasal congestion, skin redness, or a rash. After taking Xamic IV/IM orally, some may feel nauseous, have diarrhea, or experience stomach discomfort. In rare cases, Xamic IV/IM may cause low blood pressure when standing up. If an allergic reaction occurs, treatment should be stopped and suitable therapy should be started.
Pregnancy & Lactation
Xamic IV/IM should not be taken during pregnancy, as its effects on the fetus are unknown. Xamic IV/IM passes into breast milk but in very small amounts, so Xamic IV/IM is unlikely to affect the baby.
Precautions & Warnings
Xamic IV/IM should be used with caution in patients who have excessive bleeding. For surgery, Xamic IV/IM should be started 24 hours before the operation and continued for 3-4 days afterward. For bleeding, treatment should continue for at least 24 hours after the bleeding stops. Xamic IV/IM should be avoided in severe kidney problems and used cautiously in milder forms. Extra care is needed for people with heart or liver problems.
