Indications

Xefer 500mg IV is used to treat iron deficiency anemia in adults who cannot tolerate oral iron or have not responded well to Xefer 500mg IV, and in patients with chronic kidney disease who are not on dialysis.

Dosage & Administration

Step 1: Determining Iron Need: The required amount of iron depends on the patient’s body weight and hemoglobin level. The following guidelines should be followed:

  • Hemoglobin < 10 g/dl
    • Below 35 kg: 500 mg
    • 35 kg to <70 kg: 1500 mg
    • 70 kg and over: 2000 mg
  • Hemoglobin 10-14 g/dl
    • Below 35 kg: 500 mg
    • 35 kg to <70 kg: 1000 mg
    • 70 kg and over: 1500 mg
  • Hemoglobin > 14 g/dl
    • Below 35 kg: 500 mg
    • 35 kg to <70 kg: 500 mg
    • 70 kg and over: 500 mg

Step 2: Administering the Iron Dose: The dose should not exceed:

  • 15 mg of iron per kg of body weight (for intravenous injection)
  • 20 mg of iron per kg of body weight (for intravenous infusion)
  • 1000 mg of iron per administration

The maximum recommended dose is 1000 mg of iron per week.

Step 3: Post-Iron Repletion: After treatment, check the hemoglobin levels after 4 weeks. If further iron is needed, recalculate the dose based on the guidelines above.

Interaction

There have been no formal drug interaction studies performed with Xefer 500mg IV.

Contraindications

Xefer 500mg IV should not be used in people who:

  • Are allergic to the active substance or any of its ingredients
  • Have severe sensitivity to other iron injections
  • Have anemia not caused by iron deficiency
  • Have iron overload or issues with how the body uses iron

Side Effects

The side effects are generally mild and uncommon. Some possible side effects include:

  • Common: nausea, headache, dizziness, high blood pressure, injection site reactions, nausea, increased liver enzymes, low phosphate levels.
  • Uncommon: allergic reactions, heart palpitations, low blood pressure, flushing, difficulty breathing, stomach pain, diarrhea, rash, muscle pain, back pain, and fatigue.
  • Rare: serious allergic reactions, loss of consciousness, anxiety, swelling, fever, and flu-like symptoms.

Pregnancy & Lactation

There is not enough research on its use in pregnant women. Xefer 500mg IV should only be used during pregnancy if absolutely necessary. Iron from Xefer 500mg IV may pass to the fetus, so its use is restricted to the second and third trimesters when the benefits outweigh the risks. Xefer 500mg IV is unlikely to pose a risk to breastfed babies.

Precautions & Warnings

Serious allergic reactions can happen, including life-threatening issues like shock or severe low blood pressure. Patients should be closely monitored for 30 minutes after receiving the treatment. Use only when emergency care is available. Some side effects may include skin rashes, difficulty breathing, or fainting.

Hypertension: A rise in blood pressure, sometimes with dizziness or flushing, can happen after treatment. Monitor blood pressure after each dose.

Laboratory Tests: After administration, lab tests might show higher iron levels, so clinicians should account for this when interpreting results.

Overdose Effects

Overdose may cause iron to build up in the body, leading to damage. Blood tests to check for iron levels like serum ferritin can help identify overdose. Treatment may require iron removal therapy.
