Indications

Xelnib XR 11 is used to treat rheumatoid arthritis in adults who have not responded well or are unable to tolerate methotrexate. Xelnib XR 11 can be taken alone or with methotrexate or other similar medicines.

Xelnib XR 11 is also used for adults with active psoriatic arthritis who have not responded well or are unable to tolerate methotrexate or other similar medicines.

Xelnib XR 11 is used to treat adults with moderate to severe ulcerative colitis.

Pharmacology

Rheumatoid arthritis is an autoimmune disease that causes inflammation and joint damage due to abnormal immune cell activity. Tofacitinib works by blocking the signals from certain proteins in the body that cause inflammation and immune cell activation. This helps reduce inflammation and damage to the joints.

Xelnib XR 11 works by inhibiting the JAK-STAT pathway, which is important in immune cell function and the production of red blood cells. Tofacitinib may also have other ways of reducing inflammation.

Dosage & Administration

Rheumatoid Arthritis: Take 5 mg twice a day or 11 mg once a day. For those with moderate kidney or liver problems, take 5 mg once a day.

Psoriatic Arthritis: Take 5 mg twice a day or 11 mg once a day, in combination with other medications. For those with moderate kidney or liver problems, take 5 mg once a day.

Ulcerative Colitis: Take 10 mg twice a day for at least 8 weeks, then reduce to 5 or 10 mg twice a day. If no improvement is seen after 16 weeks, stop the treatment. Use the lowest effective dose to maintain the response.

Interaction

Do not take with strong inhibitors of Cytochrome P450 3A4 (e.g., Ketoconazole). If using medications that moderately affect CYP3A4 and strongly affect CYP2C19 (e.g., fluconazole), take 5 mg once a day.

Taking medications that strongly induce CYP enzymes (e.g., Rifampin) may reduce the effectiveness of this medication.

Contraindications

Xelnib XR 11 is not recommended for use in certain patients with serious infections or other health conditions, but no specific contraindications have been listed.

Side Effects

Common side effects during the first three months of treatment include upper respiratory infections, headaches, diarrhea, and nasal congestion.
