Indications
Xelpac is used for treating advanced ovarian cancer, both as the first line and after other treatments fail. Xelpac is usually combined with cisplatin for the first treatment. Xelpac is also used for breast cancer, especially when other treatments don’t work. Xelpac can be combined with drugs like doxorubicin or trastuzumab. Xelpac is used for treating non-small cell lung cancer, AIDS-related Kaposi’s Sarcoma, and gastric cancer as well.
Pharmacology
Xelpac works by helping cells form and keep microtubules, which are needed for cells to divide and function properly. Xelpac also creates abnormal clumps of microtubules that stop the cell from functioning. After being given through an IV, Xelpac spreads in the body quickly and then slowly leaves the system.
Dosage & Administration
For ovarian cancer: Paclitaxel is usually given with cisplatin in two doses: 175 mg/m² over 3 hours or 135 mg/m² over 24 hours, repeated every three weeks.
For breast cancer: The recommended dose is 175 mg/m² over 3 hours every 3 weeks for four courses, after initial chemotherapy.
For lung cancer: The dose is 175 mg/m² over 3 hours, followed by 80 mg/m² cisplatin, every three weeks.
For Kaposi’s Sarcoma: The dose is 100 mg/m² every two weeks.
Premedication with dexamethasone, diphenhydramine, and cimetidine or ranitidine is required before starting treatment.
Interaction
Xelpac may interact with other drugs like cisplatin, doxorubicin, and others. These combinations may cause more side effects, such as increased risk of kidney problems and bone marrow suppression. Also, medications that affect liver enzymes can change how paclitaxel works in the body, either increasing or decreasing its effect.
Contraindications
Xelpac should not be used by those who are allergic to Xelpac, are breastfeeding, or have low blood cell counts. Xelpac is also not recommended for people with severe liver problems or serious infections.
Side Effects
Common side effects include low blood cell counts, hair loss, joint pain, muscle pain, nausea, vomiting, diarrhea, mouth sores, and allergic reactions. Rare side effects include swelling of the feet, liver issues, and low blood pressure.
Pregnancy & Lactation
Xelpac should not be used during pregnancy or breastfeeding. Xelpac can harm the unborn baby and is not safe for use while nursing.
Precautions & Warnings
Xelpac should be given under the supervision of an experienced doctor. Hypersensitivity reactions and extravasation (leakage from the vein) are possible. Blood counts must be monitored closely, and Xelpac should not be re-administered until the blood cell counts recover. Serious heart problems and bone marrow suppression are potential risks. Xelpac should be given before cisplatin when used together.
Overdose Effects
Overdose can lead to serious side effects like bone marrow suppression, nerve damage, and mouth sores. There is no known antidote, so treatment focuses on managing the symptoms.
Storage Conditions
Xelpac should be stored in a dry place, away from light, and at temperatures below 30°C. Keep Xelpac out of reach of children and use only freshly prepared solutions.
