Indications

Xenim IV/IM is used to treat infections caused by certain harmful bacteria, including:

  • Pneumonia: caused by Streptococcus pneumoniae, Pseudomonas aeruginosa, Klebsiella pneumoniae, or Enterobacter species.
  • Febrile Neutropenia: for patients with a weakened immune system and fever.
  • Urinary Tract Infections: both uncomplicated and complicated, caused by Escherichia coli, Klebsiella pneumoniae, or Proteus mirabilis.
  • Skin and Skin Structure Infections: caused by Staphylococcus aureus or Streptococcus pyogenes.
  • Intra-abdominal Infections: when combined with metronidazole, for infections caused by Escherichia coli, Pseudomonas aeruginosa, Klebsiella pneumoniae, Enterobacter species, or Bacteroides fragilis.

Pharmacology

Xenim IV/IM works by killing bacteria. Like other antibiotics, Xenim IV/IM prevents bacteria from building their cell walls, which are necessary for their survival. Xenim IV/IM mainly affects Gram-positive bacteria, which rely heavily on their cell walls for structure.

Dosage & Administration

For Adults: Xenim IV/IM is given as an intravenous (IV) injection over about 30 minutes:

  • For pneumonia: 1-2 grams every 12 hours for 10 days.
  • For neutropenia with fever: 2 grams every 8 hours for 7 days.
  • For urinary tract infections: 0.5-1 gram every 12 hours for 7-10 days.
  • For skin infections: 2 grams every 12 hours for 10 days.
  • For intra-abdominal infections (with metronidazole): 2 grams every 12 hours for 7-10 days.

For Children: The safety and effectiveness have not been established in pediatric patients.

Interaction

Renal function should be closely monitored when using high doses with certain antibiotics (aminoglycosides), as there is an increased risk of kidney damage and hearing problems. Avoid combining with potent diuretics like furosemide.

Contraindications

Xenim IV/IM should not be used in patients who are allergic to cefepime, cephalosporins, penicillin, or other beta-lactam antibiotics.

Side Effects

Side effects are usually mild and temporary, including stomach pain, nausea, vomiting, diarrhea, and skin rash. Some patients may have allergic reactions to this medication.

Pregnancy & Lactation

Xenim IV/IM is classified as pregnancy Category B. There are no well-controlled studies for pregnant women, so Xenim IV/IM should only be used if absolutely necessary. Xenim IV/IM is excreted in breast milk in small amounts, so caution is advised when using Xenim IV/IM while breastfeeding.

Precautions & Warnings

Xenim IV/IM should not be used without clear evidence of a bacterial infection, as unnecessary use can lead to antibiotic resistance. Long-term use may cause growth of other harmful microorganisms. Patients with kidney or liver problems should use Xenim IV/IM carefully. Monitor prothrombin time in patients who are at risk of bleeding.

Overdose Effects

Overdose symptoms include confusion, hallucinations, seizures, and muscle twitching. Patients should be monitored closely and treated supportively. In cases of kidney failure, hemodialysis is recommended to remove the drug from the body.

Storage Conditions

Store in a cool, dry place and keep out of the reach of children. Do not use past the expiration date. Xenim IV/IM should only be used with a doctor’s prescription.
