Indications
Xevirol is used for treating postmenopausal women with advanced hormone receptor-positive, HER2-negative breast cancer when treatment with letrozole or anastrozole has failed, in combination with exemestane.
Xevirol is also used for adults with progressive neuroendocrine tumors (PNET) of pancreatic origin and those with advanced, non-functional neuroendocrine tumors (NET) in the gastrointestinal (GI) or lung area that cannot be surgically removed, are locally advanced, or have spread.
Xevirol is used for adults with advanced renal cell carcinoma (RCC) after treatment with sunitinib or sorafenib has failed.
Xevirol is also prescribed for adults with renal angiomyolipoma and tuberous sclerosis complex (TSC) who do not need immediate surgery.
Xevirol is used for treating patients aged 1 year and older with TSC who have a specific brain tumor called subependymal giant cell astrocytoma (SEGA) that cannot be removed surgically.
Xevirol is used to treat adults and children aged 2 years and older with TSC-related partial-onset seizures.
Pharmacology
Xevirol is an inhibitor of mTOR, a protein involved in cell growth, which plays a role in several cancers and TSC. Xevirol works by blocking the mTOR pathway, reducing cell growth and affecting proteins needed for cancer cells to multiply.
Xevirol reduces cell growth, blood vessel growth, and glucose uptake, making Xevirol useful for treating tumors. Studies show Xevirol can help treat certain types of breast cancer, including estrogen-dependent and HER2-positive breast cancer cells, when used with other drugs.
Dosage & Administration
Breast Cancer: The recommended dosage is 10 mg taken orally once a day until disease progression or unacceptable side effects occur.
Neuroendocrine Tumors (NET): The recommended dosage is 10 mg taken orally once a day until disease progression or unacceptable side effects occur.
Renal Cell Carcinoma (RCC): The recommended dosage is 10 mg taken orally once a day until disease progression or unacceptable side effects occur.
Tuberous Sclerosis Complex (TSC) – Renal Angiomyolipoma: The recommended dosage is 10 mg taken orally once a day until disease progression or unacceptable side effects occur.
TSC-Associated SEGA: The recommended starting dosage is 4.5 mg/m² taken orally once a day until disease progression or unacceptable side effects occur.
TSC-Associated Partial-Onset Seizures: The recommended starting dosage is 5 mg/m² taken orally once a day until disease progression or unacceptable side effects occur.
Interaction
Avoid using Xevirol with strong P-gp inhibitors and CYP3A4 inhibitors.
If using with moderate P-gp or CYP3A4 inhibitors, reduce the dose as needed.
If using with P-gp or strong CYP3A4 inducers, increase the dose as recommended.
Contraindications
Xevirol is contraindicated for patients who are allergic to Xevirol or other similar drugs.
Side Effects
Common side effects for patients with breast cancer, NET, and RCC include mouth sores, infections, rash, fatigue, diarrhea, swelling, abdominal pain, nausea, fever, weakness, cough, headache, and loss of appetite.
For patients with TSC-related renal angiomyolipoma, the most common side effect is mouth sores.
For patients with TSC-related SEGA, the most common side effects are mouth sores and respiratory infections.
For patients with TSC-related partial-onset seizures, the most common side effect is mouth sores.
Pregnancy & Lactation
Xevirol can harm the fetus if used during pregnancy. There are limited reports of its use in pregnant women, but they don’t provide enough information about birth defects or miscarriage risks. Xevirol is not known if Xevirol passes into breast milk, so talk to your doctor before breastfeeding.
Precautions & Warnings
Monitor for lung problems, infections, severe allergic reactions, and mouth sores. Discontinue treatment if these problems are severe. Be cautious about using Xevirol before surgery or with other medications. Regular monitoring of kidney function and blood tests is necessary.
Avoid live vaccines and contact with people who have received live vaccines. Xevirol can affect wound healing, so Xevirol should be paused before and after surgery. Women who are pregnant or planning to become pregnant should be advised of its risks and use effective contraception.
