Indications

It is used to treat traveler’s diarrhea caused by noninvasive E. coli strains, reduce the risk of hepatic encephalopathy, and treat bacterial overgrowth in irritable bowel syndrome.

Pharmacology

It is a semi-synthetic, rifamycin-based antibiotic that doesn’t get absorbed much into the bloodstream. It works by blocking bacterial RNA production, which helps kill harmful bacteria. It is effective against many types of Gram-positive and Gram-negative bacteria, as well as both aerobic and anaerobic bacteria.

Dosage & Administration

Traveler’s Diarrhea: For patients ≥12 years old: 200 mg 3 times a day for 3 days.

Hepatic Encephalopathy: For patients ≥18 years old: 550 mg 2 times a day.

Bacterial Overgrowth in Irritable Bowel Syndrome: 400 mg 3 times a day for 10 days or 550 mg 3 times a day for 14 days. Can be taken with or without food.

Interaction

Studies suggest that it might increase the activity of the CYP3A4 enzyme. However, in patients with normal liver function, the recommended dose is not expected to cause this effect.

Contraindications

It should not be used by patients who are allergic to Rifaximin or any of the rifamycin antibiotics or any of its ingredients.

Side Effects

Side effects may include gas, headache, stomach pain, rectal discomfort, urgency to defecate, nausea, constipation, fever, and vomiting. Severe reactions such as anaphylaxis, swelling, and skin irritation have been reported.

Pregnancy & Lactation

It is classified as pregnancy category C. It is not known if Rifaximin is passed into breast milk.

Precautions & Warnings

It is not effective in patients with diarrhea caused by fever or blood in the stool. If symptoms worsen or persist for more than 24-48 hours, Rifaximin should be stopped, and another antibiotic should be considered. It may cause a serious gut infection called pseudomembranous colitis, so doctors should be cautious when prescribing it for patients with diarrhea after using antibiotics.

Overdose Effects

There is no specific treatment for an overdose. If an overdose occurs, stop taking Rifaximin and treat the symptoms as necessary with supportive care.
