Indications

Xorubin is used as part of a combination treatment for women with breast cancer that has spread to the lymph nodes after surgery. Xorubin is also used to treat various cancers including leukemia, lymphoma, breast cancer, neuroblastoma, bone and soft tissue sarcomas, ovarian cancer, bladder cancer, thyroid cancer, stomach cancer, and lung cancer.

Pharmacology

Xorubin is a cancer treatment drug that works by interfering with the growth of cancer cells. Xorubin affects the DNA in cells, stopping them from growing and dividing. This process helps to kill cancer cells. Xorubin is especially effective because Xorubin targets the enzyme responsible for DNA replication, which is crucial for cancer cell survival.

Dosage & Administration

Single agent: 60 to 75 mg/m² given through an IV every 21 days.

In combination therapy: 40 to 75 mg/m² given through an IV every 21 to 28 days. If the patient has heart problems or liver issues, the dose may be adjusted. For IV administration, Xorubin is injected through a central line or a secure peripheral line using specific solutions like sodium chloride or dextrose. The injection should be slow to avoid side effects like skin irritation or facial flushing. If there are any signs of fluid leakage during injection, the treatment should be stopped immediately, and special care should be taken to manage the situation.

Interaction

Xorubin is processed in the body by certain enzymes (CYP3A4, CYP2D6, and P-glycoprotein). Using other drugs that affect these enzymes can change how well Xorubin works. Specifically, using Xorubin with Trastuzumab can increase the risk of heart problems. If both Paclitaxel and Xorubin are used together, the dose of Xorubin should be given first as Paclitaxel can increase the levels of Xorubin in the body.

Contraindications

Xorubin is not suitable for patients with severe heart problems, recent heart attacks, extreme suppression of blood cells, severe liver damage, or a history of severe allergic reactions to the drug.

Side Effects

Common side effects include hair loss, nausea, and vomiting. Other effects may include heart issues, irregular heartbeats, secondary cancers, skin problems at the injection site, low blood cell counts, and reactions to radiation therapy.

Pregnancy & Lactation

Xorubin is harmful to a developing baby and should be avoided during pregnancy unless absolutely necessary. Women who can become pregnant should use reliable contraception during treatment and for six months after. Xorubin can also pass into breast milk, so breastfeeding should be stopped if the drug is being used.

Precautions & Warnings

Extra care is needed for patients who might have iron overload or conditions like certain types of anemia. If the treatment is not effective, the underlying cause should be checked further.

Overdose Effects

Taking too much can cause serious heart problems, blood issues, and may even be fatal. Overdose effects can be delayed, showing up between 10 to 15 days after the dose. Careful monitoring is needed, and treatment will focus on supporting the patient.
