Indications
Xpos 100 DR is used to treat certain fungal infections in patients 13 years and older, including:
- Invasive aspergillosis in patients who do not respond to or cannot tolerate Amphotericin B or Itraconazole.
- Fusariosis in patients who do not respond to or cannot tolerate Amphotericin B.
- Chromoblastomycosis and mycetoma in patients who do not respond to or cannot tolerate Itraconazole.
- Coccidioidomycosis in patients who do not respond to or cannot tolerate Amphotericin B, Itraconazole, or Fluconazole.
Xpos 100 DR is also used to prevent fungal infections in high-risk patients, such as:
- Patients undergoing chemotherapy for Acute Myelogenous Leukemia (AML) or Myelodysplastic Syndromes (MDS) that causes prolonged low white blood cell count.
- Patients who have received a stem cell transplant and are undergoing high-dose immunosuppressive therapy for graft versus host disease.
Pharmacology
Xpos 100 DR works by blocking the production of ergosterol, an important part of the fungal cell membrane. Xpos 100 DR prevents the enzyme that changes lanosterol into ergosterol, leading to a build-up of other compounds and a lack of ergosterol. This weakens the fungal cell membrane, affecting its function and structure.
Dosage & Administration
Adult & Elderly: Take one tablet (300 mg) twice a day on the first day. From the second day, take one tablet (300 mg) once a day. You can take the tablets with or without food. Do not crush, chew, or break the tablets. The treatment duration depends on the severity of the infection and how the patient responds.
Pediatric patients: Xpos 100 DR is not recommended for children under 13 years old due to safety concerns.
Interaction
Xpos 100 DR can interact with other medicines metabolized by the liver enzyme CYP3A4, such as Midazolam, Triazolam, and Alprazolam. Xpos 100 DR may lower its own levels when taken with antibiotics like Rifampicin and anticonvulsants such as Phenytoin. Always check with a healthcare provider about possible interactions.
Contraindications
Xpos 100 DR should not be used by people who are allergic to Xpos 100 DR or other azole antifungal drugs. Xpos 100 DR is also not recommended for patients taking certain medications like ergot alkaloids, some antipsychotics, and cholesterol-lowering drugs such as simvastatin, lovastatin, or atorvastatin.
Side Effects
Common side effects include diarrhea, nausea, vomiting, headache, fever, coughing, and low potassium levels (hypokalemia).
Pregnancy & Lactation
Xpos 100 DR should only be used during pregnancy if the benefits to the mother outweigh the risks to the baby. Breastfeeding should be stopped while using this medicine.
