Indications
It is used to treat moderate to severe pain, including pain after surgery, colic, spastic pain, cancer pain, joint pain, neck and back pain, and pain caused by osteoporosis.
Pharmacology
It is a pain-relieving drug that works in the brain. It helps block pain by increasing the levels of serotonin and noradrenaline, which are chemicals that control pain. It also works through opioid receptors to reduce pain and has a role in blocking other pain pathways in the body.
Dosage & Administration
Capsule or Tablet: The usual dose is 50 to 100 mg every four to six hours. For acute pain, the initial dose is 100 mg, and for chronic pain, 50 mg is recommended to start. The total daily dose should not exceed 400 mg.
Sustained Release Capsule or Tablet: One capsule or tablet every 12 hours. The total daily dose should not exceed 400 mg.
Injection: A dose of 50-100 mg every 4 to 6 hours through intramuscular injection or intravenous infusion. The total daily dose should not exceed 600 mg.
Suppository: The usual dose is 100 mg every 6 hours, but for cancer pain or severe pain after surgery, higher doses can be used.
Interaction
Generally, there are no major concerns with other drugs interacting with it, except for monoamine oxidase (MAO) inhibitors, which should not be used together. If used with carbamazepine, the dose of Tramadol may need to be increased.
Contraindications
It is not recommended for people who are allergic to it or who have been intoxicated by alcohol, sedatives, other pain relievers, or psychotropic drugs.
Side Effects
Common side effects include dizziness, nausea, constipation, headaches, drowsiness, vomiting, itching, weakness, sweating, upset stomach, dry mouth, and diarrhea. Less common effects may include fatigue, allergic reactions, weight loss, heart palpitations, stomach pain, loss of appetite, bloating, and gastrointestinal issues.
Pregnancy & Lactation
The safety of it during pregnancy is not well established. It should only be used during pregnancy if the benefits outweigh the risks. It should not be used while breastfeeding as it can pass into breast milk.
Precautions & Warnings
Caution is needed when used in people at risk for breathing problems, those who are dependent on opioids, or those taking other drugs that affect the central nervous system. It should also be used carefully in people with high pressure inside the skull, head injuries, or acute stomach issues.
Overdose Effects
The effects of an overdose can vary, but may include abdominal pain, nausea, vomiting, diarrhea, black or bloody stools, dizziness, and even seizures, unconsciousness, and shock.
