Indications
Xylogel 2% is indicated for surface anesthesia of the male and female urethra, treatment of painful urethritis, as an anesthetic lubrication for endotracheal intubation, and to provide adequate pain relief during endoscopy.
Pharmacology
Xylogel 2% works by blocking the flow of sodium through sodium channels in nerve cells. This prevents the nerve from sending signals and causes a numbing effect. Lidocaine binds more easily to sodium channels when the nerve is active, making Xylogel 2% faster at blocking nerve signals in quickly firing nerves.
Dosage & Administration
Urethral Anesthesia:
For male urethra: 400 mg for sufficient pain relief.
For female urethra: Instill 100-200 mg in small amounts to cover the entire urethra, allowing a few minutes for full numbing before any procedure.
Lubrication for Endotracheal Intubation: Apply 100 mg on the surface of the tube just before insertion, being careful not to let the jelly enter the tube’s lumen.
Endoscopy: Instill 200-400 mg for adequate pain relief, applying a small amount of jelly to lubricate the instrument. For debilitated, elderly patients, and children, adjust doses according to their condition.
Note: Sterilize the applicator by boiling Xylogel 2% for 5 minutes, then attach to the tube and slowly instill the jelly as needed. Detach the applicator after each use and ensure the tube is tightly closed.
Interaction
Xylogel 2% should be used with caution in patients who are also taking antiarrhythmic drugs like tocainide, as the toxic effects may increase.
Contraindications
Xylogel 2% is contraindicated in patients who have a known allergy to local anesthetics of the amide type.
Side Effects
Side effects may include nervousness, dizziness, blurred vision, tremors, drowsiness, convulsions, unconsciousness, breathing problems, low blood pressure, heart problems, and allergic reactions such as skin rashes, swelling, or severe reactions like anaphylaxis.
Pregnancy & Lactation
There is limited information on the safety of this drug during pregnancy, but Xylogel 2% has been used for many years without apparent harm. Xylogel 2% can be used during pregnancy if no safer alternatives are available. Xylogel 2% passes into breast milk in small amounts, but this is usually not a concern at standard doses.
Precautions & Warnings
Be cautious when using Xylogel 2% on damaged skin or mucous membranes, as absorption may be higher. Xylogel 2% should also be used carefully in patients with epilepsy, heart issues, low heart rate, liver problems, or severe shock. Numbness in the mouth or throat can increase the risk of choking, especially in children.
