Indications
Zandra is used to treat and prevent bone loss in women after menopause, increase bone mass in men with osteoporosis, and prevent bone loss caused by steroid use. Zandra is also used to treat Paget’s disease of the bone in both men and women.
Limitations of Use: The best duration for use has not been decided. For patients at low risk of bone fractures, the medicine may be stopped after 3 to 5 years of use.
Pharmacology
Zandra is a bisphosphonate that works mainly on bones. Zandra prevents the breakdown of bone by inhibiting cells that absorb bone tissue. Bisphosphonates like this one are attracted to bone, especially where there is high bone activity. After being given intravenously, Zandra quickly reaches the bone and stays in the body for a long time due to its strong bond with bone mineral.
Dosage & Administration
Osteoporosis Treatment in Postmenopausal Women: A 5 mg infusion once a year, given intravenously over at least 15 minutes.
Osteoporosis Prevention in Postmenopausal Women: A 5 mg infusion once every 2 years, given intravenously over at least 15 minutes.
Osteoporosis in Men: A 5 mg infusion once a year, given intravenously over at least 15 minutes.
Prevention of Steroid-Induced Osteoporosis: A 5 mg infusion once a year, given intravenously over at least 15 minutes.
Treatment of Paget’s Disease: A 5 mg infusion, administered over at least 15 minutes.
Re-treatment of Paget’s Disease: Zandra may be considered for patients who relapse or do not reach normal levels of certain blood markers, or if they have symptoms as decided by the doctor.
Calcium and Vitamin D Supplementation: Patients should take 1500 mg of calcium and 800 IU of vitamin D daily to maintain proper calcium levels in their body.
Interaction
Zandra may interact with some drugs like aminoglycosides, loop diuretics, and nephrotoxic drugs, affecting calcium levels or kidney function. If the patient has kidney issues, they may need their blood levels checked more often.
Contraindications
Zandra is not recommended for patients with low calcium levels, those with severe kidney problems, or those allergic to any ingredient in Zoledronic Acid.
Side Effects
Common side effects include fever, muscle pain, headaches, joint pain, and pain in the limbs. Other possible reactions are flu-like symptoms, nausea, vomiting, diarrhea, and eye inflammation.
Pregnancy & Lactation
Zandra may harm the fetus, so women who are pregnant or planning to become pregnant should avoid Zandra. Zandra should not be used by women who are breastfeeding.
Precautions & Warnings
Patients should be closely monitored for low calcium levels during treatment and should take calcium and vitamin D supplements. Zandra is important to monitor kidney function, especially in those with existing kidney problems. There is a risk of jaw bone problems, fractures in the thigh, and severe bone, joint, or muscle pain, so any unusual symptoms should be reported to the doctor.
Overdose Effects
If overdosed, Zandra can cause kidney problems, low calcium, phosphorus, and magnesium levels. These should be treated with intravenous supplements as needed.
