Indications

Zapenia 25 is used to treat schizophrenia in patients who do not respond to or cannot tolerate regular antipsychotic drugs. Zapenia 25 is also used for psychosis in Parkinson’s disease.

Pharmacology

Zapenia 25 is an ‘atypical’ antipsychotic drug because Zapenia 25 works differently from other typical antipsychotics. Zapenia 25 interacts with various dopamine receptors and has a higher effect on limbic receptors compared to striatal ones, which may explain why Zapenia 25 has fewer side effects like movement disorders. Zapenia 25 also works on other receptors, including adrenergic, cholinergic, histaminergic, and serotonergic ones.

Dosage & Administration

Schizophrenia: Adults over 16 years start with 12.5 mg once or twice on the first day (12.5 mg for elderly) and increase the dose over the following days in steps of 25-50 mg daily until the dose reaches 300 mg. If necessary, the dose may be increased further to a maximum of 900 mg daily.

Psychosis in Parkinson’s disease: Start with 12.5 mg at bedtime and increase the dose gradually in steps of 12.5 mg up to 50 mg daily. The maximum dose may be increased to 100 mg daily in 1-2 doses if needed.

Interaction

Zapenia 25 is not recommended to use with other drugs that suppress the bone marrow. Zapenia 25 may interact with CNS-active drugs and alcohol, and cause low blood pressure when used with certain drugs. Zapenia 25 may also affect the levels of other protein-bound drugs like warfarin and digoxin. Be cautious when using with Carbamazepine, fluvoxamine, paroxetine, or sertraline. Avoid using adrenaline with this drug.

Contraindications

Zapenia 25 is not suitable for people with severe heart problems, kidney issues, a history of blood disorders, or certain types of psychoses. Zapenia 25 should also be avoided during breastfeeding.

Side Effects

Common side effects include constipation, dizziness, drowsiness, headache, mouth watering, nausea, vomiting, and weight gain. Less common effects include stomach discomfort and dry mouth.

Pregnancy & Lactation

There is no clear evidence of effects in pregnant women, but Zapenia 25 may be used during pregnancy if necessary. Zapenia 25 may pass into breast milk, so Zapenia 25 is not recommended for breastfeeding women.

Precautions & Warnings

Do not stop taking Zapenia 25 suddenly; Zapenia 25 should be tapered off gradually. Zapenia 25 may cause serious blood problems, so regular blood tests are required. Zapenia 25 may also cause sleepiness and affect the ability to perform tasks requiring alertness, like driving. Use with caution in people with a history of seizures, heart disease, diabetes, or liver issues.

Overdose Effects

Overdose symptoms include drowsiness, confusion, fast heart rate, low blood pressure, difficulty breathing, and excessive salivation. In severe cases, Zapenia 25 may cause heart problems, seizures, or even death. Medical attention should be sought immediately.
