Indications
Zenilac is used to treat constipation, especially when other methods like a fiber-rich diet, enough fluids, and physical activity don’t work. Zenilac is also used to help when the balance of bacteria in the intestines is disrupted, such as after long-term antibiotic use, or in conditions like gallbladder diseases, colitis, diverticulosis, and megacolon. Additionally, Zenilac helps reduce high blood ammonia levels, which can occur in liver disease and certain brain conditions.
Pharmacology
Zenilac is a synthetic sugar that works in the colon. Bacteria in the colon break Zenilac down, creating acids that lower the pH of the colon and help retain water, which increases bowel movements. Most of Zenilac is not absorbed into the body, so Zenilac mainly works in the intestines.
Dosage & Administration
For constipation (chronic constipation):
Adults: Start with 3-6 teaspoons daily, then reduce to 1½-6 teaspoons daily for long-term use.
Children up to 14 years: Start with 3 teaspoons daily, then reduce to 1-2 teaspoons daily for long-term use.
Infants and toddlers: Start with 1-2 teaspoons daily, then reduce to 1 teaspoon daily for long-term use.
For damaged intestinal flora:
Adults: 1-2 teaspoons daily.
Children: 1 teaspoon daily.
For reducing blood ammonia levels:
For hyper-ammoniemia in liver disease: Up to 18-30 teaspoons daily.
For portal-systemic encephalopathy: 6-9 teaspoons every hour to induce quick bowel movement. Once effective, the dose can be reduced.
Interaction
Zenilac has no significant drug interactions. However, using Zenilac with cardiac medications can increase potassium loss, which may affect heart function.
Contraindications
Zenilac is not recommended for people who are allergic to galactose or lactose, or who need to avoid galactose in their diet. Zenilac should also be avoided in people with certain digestive conditions or suspected bowel obstruction.
Side Effects
Zenilac can cause some gas, cramps, or stomach discomfort, especially at the beginning of treatment. These usually go away if the dose is reduced. Overdose can lead to diarrhea. If used too much, Zenilac may cause a loss of important electrolytes like potassium.
Pregnancy & Lactation
Zenilac is categorized as pregnancy Category B by the FDA, meaning studies show Zenilac is safe during pregnancy. However, decisions about using Zenilac during pregnancy and breastfeeding should be made by a doctor.
Precautions & Warnings
Zenilac should be used carefully by people who are intolerant to lactulose. In cases of severe liver disease, higher doses are used, which should be considered in people with diabetes.
Overdose Effects
Although there have been no reports of accidental overdose, if too much is taken, diarrhea and stomach cramps are expected.
