Indications
Zeptol CR 400 is used to treat:
- Partial and secondary generalized tonic-clonic seizures
- Primary generalized tonic-clonic seizures
- Trigeminal neuralgia
- Prevention of bipolar disorder
Pharmacology
Zeptol CR 400 works by reducing activity in a part of the brain, limiting the flow of sodium ions into cells, and using other methods that are not fully understood. Zeptol CR 400 also helps in water reabsorption by stimulating the release of a hormone that regulates water balance in the body.
Dosage & Administration
For Epilepsy:
- Adults & children over 12 years: Start with 200 mg twice a day (tablets) or 1 teaspoon four times a day (suspension). Gradually increase by 200 mg each week.
- Children 12-15 years: Dosage generally should not exceed 1000 mg daily. The maintenance dose is usually 800-1200 mg daily.
- Children 6-12 years: Start with 100 mg twice a day or 1/2 teaspoon four times a day (200 mg/day). Increase as needed.
- Children under 6 years: Start with 10-20 mg per kg of body weight per day. The daily dose should not exceed 35 mg per kg.
For Trigeminal Neuralgia:
- Start with 100 mg twice a day and increase by 100 mg every 12 hours as needed, up to a maximum of 1200 mg daily.
- Most patients can maintain pain relief with 400-800 mg daily, but some may require up to 1200 mg daily.
Interaction
Taking this medicine with oral contraceptives may cause changes in hormone levels. Also, other drugs like Phenytoin may increase its metabolism, meaning a higher dose might be needed.
Contraindications
Zeptol CR 400 should not be used by people who:
- Are allergic to any of its ingredients
- Have heart problems (atrioventricular block)
- Have bone marrow depression
- Have porphyrias (a type of blood disorder)
- Have allergies to tricyclic antidepressants
- Have taken a monoamine-oxidase inhibitor antidepressant (MAOI) in the past 14 days
Side Effects
The common side effects include dizziness, drowsiness, unsteady movements, dry mouth, stomach pain, nausea, vomiting, loss of appetite, and slow heart rate. More serious effects can include a skin rash or low blood cell count.
Pregnancy & Lactation
Zeptol CR 400 is classified as Pregnancy Category D. Zeptol CR 400 is transferred to breast milk, so a decision should be made whether to stop nursing or the medication based on potential risks.

Zeptol CR 400 can decrease the effectiveness of birth control pills, so a stronger contraceptive may be needed. Do not stop taking Zeptol CR 400 suddenly unless your doctor advises Zeptol CR 400, as this could make symptoms return.
Special caution is required for people with a history of mixed seizures, heart problems, kidney or liver disease, and certain blood disorders.
