Indications
Zoton 30 is used for the short-term treatment of active duodenal ulcer, maintenance of healed duodenal ulcers, short-term treatment of active benign gastric ulcers, short-term treatment of active erosive esophagitis, and maintenance of healing of erosive esophagitis. Zoton 30 is also used for pathological conditions with excessive acid secretion, including Zollinger-Ellison Syndrome, and to help eradicate H. pylori to reduce the risk of duodenal ulcer recurrence.
Pharmacology
Zoton 30 is a type of medication known as a proton pump inhibitor (PPI). Zoton 30 works by blocking the final step of acid secretion in the stomach, specifically by inhibiting the H+/K+ ATPase enzyme system in the stomach cells. This reduces both basal and stimulated acid production in the stomach.
Dosage & Administration
Benign gastric ulcer: 30 mg daily in the morning for 8 weeks.
Duodenal ulcer: 30 mg daily in the morning for 4 weeks; maintenance dose 15 mg daily.
NSAID-associated ulcers: 15-30 mg daily for 4 weeks, and an additional 4 weeks if not fully healed.
Zollinger-Ellison syndrome: Initially 60 mg daily, adjusted based on response. Doses over 120 mg per day should be divided into two doses.
Gastroesophageal reflux disease (GERD): 30 mg daily in the morning for 4 weeks, with another 4 weeks if not fully healed; maintenance dose 15-30 mg daily.
Acid-related dyspepsia: 15-30 mg daily in the morning for 2-4 weeks.
Interaction
Zoton 30 may affect the cytochrome P-450 system, which can influence how the liver processes other drugs, although no significant interactions have been reported. There is some uncertainty about its effect on combined oral contraceptives. Reducing stomach acid may also affect vitamin B12 absorption and bacterial colonization in the stomach and intestines.
Contraindications
Zoton 30 is contraindicated in patients who are allergic to any component of the formulation.
Side Effects
Severe side effects may include a possible increase in the risk of carcinoid tumors due to long-term acid suppression and a rise in serum gastrin levels. There may also be an increased risk of atrophic gastritis with long-term use in patients with H. pylori infection. Common side effects include diarrhea (which is dose-dependent), headache, and occasional stomach discomfort.
Pregnancy & Lactation
Zoton 30 should be avoided during pregnancy unless necessary for the mother’s health. The potential risks and benefits should be carefully considered. Zoton 30 is not known whether Lansoprazole passes into breast milk, so Zoton 30 should be used cautiously during breastfeeding.
Precautions & Warnings
Gastric cancer should be ruled out before starting treatment. Caution is needed in patients with liver problems and those who are pregnant or breastfeeding.
