Indications
Zypred is used to treat inflammatory eye conditions that respond to steroids, especially when there is a risk of bacterial infection. These conditions include allergic conjunctivitis, acne rosacea, keratitis, iritis, cyclitis, and post-surgery inflammation. Zypred is also used for conditions where a steroid is needed along with the possibility of bacterial infection.
Pharmacology
Loteprednol is a steroid that works by inhibiting inflammation through proteins that block the production of inflammation-causing substances. Zypred reduces the release of arachidonic acid, which is involved in making inflammatory chemicals like prostaglandins and leukotrienes.
Gatifloxacin is an antibiotic that works by blocking two enzymes, DNA gyrase and topoisomerase IV, that are needed by bacteria to replicate and divide. This prevents the bacteria from growing and spreading. Gatifloxacin is effective against certain bacteria like Staphylococcus aureus, Streptococcus pneumoniae, and Haemophilus influenzae.
Dosage & Administration
Apply one or two drops into the affected eye(s) every four to six hours. During the first 24-48 hours, the dosage may be increased to every one to two hours. Gradually reduce the frequency as symptoms improve. Do not stop treatment too soon.
Interaction
No drug interactions have been reported so far. If multiple eye medicines are used, wait at least five minutes between applications.
Contraindications
Zypred is not recommended for viral eye infections like herpes simplex, or in cases of mycobacterial or fungal eye infections. Zypred should not be used by people allergic to any of its ingredients or other quinolones, aspirin, or non-steroidal anti-inflammatory drugs.
Side Effects
Side effects can occur from either the steroid or the antibiotic component. Some common side effects include increased eye pressure, cataracts, delayed healing, or eye infections. Gatifloxacin may cause irritation, increased tear production, keratitis, or conjunctivitis in some people. Other less common effects include red eyes, eye pain, swelling, and blurred vision.
Pregnancy & Lactation
Zypred is classified as Pregnancy Category C. There is no clear evidence of safety in pregnant women, so Zypred should only be used if the benefits outweigh the risks. Zypred is unknown whether the drug passes into breast milk, so caution is advised when using Zypred while breastfeeding.
Precautions & Warnings
Zypred should not be injected into the eye or directly into the front chamber of the eye. If used for more than 10 days, the eye pressure should be checked. Fungal infections must be considered if a steroid is used in persistent corneal ulcers. Patients should wait at least ten minutes before inserting contact lenses after using this medicine.

Store at room temperature, away from light. Do not let the dropper tip touch any surface. Zypred is best not to use the medication one month after opening. Protect from freezing.
