Indications
Abiraterone Acetate is indicated in combination with prednisone for the treatment of:
- Metastatic castration-resistant prostate cancer (CRPC)
- Metastatic high-risk castration-sensitive prostate cancer (CSPC)
Pharmacology
Abiraterone Acetate is converted in vivo to Abiraterone, an androgen biosynthesis inhibitor that inhibits 17α-hydroxylase/C17, 20-lyase (CYP17). This enzyme, required for androgen biosynthesis, is expressed in testicular, adrenal, and prostatic tumor tissues. CYP17 catalyzes two reactions:
- Conversion of pregnenolone and progesterone to their 17α-hydroxy derivatives.
- Formation of dehydroepiandrosterone (DHEA) and androstenedione, precursors of testosterone.
Inhibition of CYP17 reduces androgen levels, essential for managing androgen-sensitive prostatic carcinoma. Additionally, Zytix 250 increases mineralocorticoid production, requiring monitoring for associated side effects.
Dosage & Administration
Metastatic CRPC: 1,000 mg (two 500 mg tablets or four 250 mg tablets) once daily with Prednisone 5 mg twice daily.
Metastatic High-Risk CSPC: 1,000 mg once daily with Prednisone 5 mg once daily.
Patients should also receive a GnRH analog or have had bilateral orchiectomy. Take on an empty stomach (at least 1 hour before or 2 hours after a meal). Swallow tablets whole with water.
